NHS-funded reablement: what 6 weeks free actually covers
Almost every family hears 'the first 6 weeks are free'. Here is exactly what that means, what it does not cover, and what happens at week 7.
The short answer. The first up to 6 weeks of reablement are free under the NHS regardless of means, when commissioned as part of Discharge to Assess or NICE NG74 intermediate care. It covers care visits, therapy, equipment and short-term bed placement if needed. It does not cover long-term care, top of preference homes, or extras. Week 7 onwards switches to standard council means-testing or self-funding.
What is included
- Care visits (typically 3-4 per day for the first fortnight).
- Occupational therapy home assessment and equipment.
- Physiotherapy if mobility is the goal.
- Bed-based reablement placement (Pathway 2) if home is not safe.
- Nursing input where needed.
What is excluded
- Long-term care (week 7 onwards).
- Personal extras (hairdressing, chiropody, newspapers).
- Top-up for a preferred home.
- Care that is purely social (companionship, shopping) without rehabilitative goal.
In a care home or only at home?

Who funds week 7
Standard rules apply from week 7. Below the upper capital limit (£23,250 in England 2026/27) the council pays the contract rate. Above it, the patient is a full self-funder. NHS Continuing Healthcare may be granted if a primary health need is established.

Is the 6 weeks fixed?
It is a guide. NICE NG74 supports extension where reablement is still showing progress. Always ask the OT lead for a written extension if you can see continued gains.
Does reablement always work?
Roughly two-thirds of patients show meaningful gains. The point of the model is to test, not to assume long-term care. A failed reablement is a valid assessment outcome, not a personal failure.
Can I top up reablement with private care?
Yes. Many families use a paid carer to fill gaps between NHS visits. This is private spending and does not affect the NHS reablement entitlement.
